Job summary

Montgomery County, PA is seeking an Early Intervention Service Coordinator to manage a caseload and help eligible families identify and access mandated services for children birth to three years.

Responsibilities include screening referrals, coordinating the Multi-Disciplinary Evaluation within 45 days, facilitating IFSP and transition plans, monitoring health and safety concerns, and assisting families in obtaining medical and community resources.

Qualifications

• Bachelor’s degree with 12 college credits in early intervention, early childhood, special education, child development, family counseling or related social sciences.
• One year of full-time or equivalent experience working with or providing counseling to children, families or individuals with disabilities.
• Mandates requiring Child Abuse, Criminal History, and FBI Clearances for employment; PA residency not required.

Responsibilities

• Screen referrals to determine resources, priorities and concerns of the family.
• Coordinate Multi-Disciplinary Evaluation within 45 days of initial referral and assist in IFSP development and transition planning.
• Facilitate and participate in IFSP development, implementation, review and evaluation; monitor health and safety concerns.
