Qualifications

• Valid/active Kentucky Registered Nurse License or eligibility from a Nursing Compact State.
• Minimum one year experience with elderly population as an RN.
• Minimum one year experience in case management as an RN.
• Valid driver's license and reliable transportation required.
• Ability to pass comprehensive background check and drug screen.

Responsibilities

• Perform non-clinical, in-home assessments on a weekly basis.
• Manage a dedicated case load with flexible scheduling.
• Complete all documentation electronically through a secure portal.
