Qualifications

• Minimum of 2 years’ experience in social services, human services, healthcare, or child welfare.

• Bilingual preferred is a plus; excellence in communication and organizational skills required.

Responsibilities

• Intake, triage, and risk assessments: manage and assess all incoming intakes from statewide line, community partners, DCF, law enforcement, hospitals, and schools.
• Cultivate key partnerships with DCF, law enforcement, hospitals, and schools; educate partners on programs and referral criteria.
• Data management and follow-up: track and document intake information, maintain records, and ensure confidentiality and compliance.
