Responsibilities include outreach, eligibility assessments, and ensuring compliant, data-driven enrollments while delivering compassionate patient care and clear program information.

Qualifications

• Strong written and verbal communication skills.
• Empathy and the ability to connect with patients and families.
• Comfortable using technology for documentation and communication.
• Sales or customer service experience is a plus but not required.
• Residence in the US as a operating requirement.

Responsibilities

• Outreach to eligible CCM/RPM patients to initiate enrollment.
• Explain benefits of CCM/RPM programs clearly and compassionately to patients and families.
• Assist eligible patients with the enrollment process and complete documentation.
• Maintain accurate records of patient interactions, enrollment status, and program details.
• Stay up-to-date with CCM/RPM compliance requirements and ensure ethical enrollment.
