Job summary

Think Whole Person Healthcare Aksarben is seeking a Nurse Coordinator to manage care coordination for Think patients, serving as the primary conduit to the clinical team and developing patient-specific plans within value-based programs.

The role collaborates with multidisciplinary teams to optimize resources, improve health outcomes, and reduce costs, with a trajectory toward a more personalized care approach and high-quality patient experiences.

Qualifications

• ADN required; BSN preferred; Nebraska RN license required.
• Active BLS/CPR certification required.
• Minimum 2 years nursing experience; BSN with 2 years case/care management preferred.
• Experience in triage setting preferred.

Responsibilities

• Assess the medical, social, and psychological needs of patients.
• Create patient-specific care plans and identify resources.
• Provide education on various healthcare conditions to improve outcomes.
• Maintain detailed documentation in the Electronic Health Record (EHR).
• Coordinate with service providers to fulfill patient needs.
• Communicate with patients by phone per internal protocols.
• Plan routine or acute visits with providers and obtain required information.
• Review external clinical data and delegate to appropriate team members.
• Coordinate care with nursing homes, home health agencies, and hospitals.
• Schedule transitions of care visits and outgoing calls.
• Delegate tasks to LPN, MA, and other staff as needed.
