Qualifications

• Active LPN license in South Carolina is required.
• Experience in home health, long-term care, or clinical nursing is preferred.
• Strong knowledge of medications, wound care, and patient assessment.
• Excellent communication with patients and families.

Responsibilities

• Monitor and document patient health status, vital signs, and progress toward care goals.
• Communicate effectively with patients, families, and the interdisciplinary care team.
• Educate patients and caregivers on disease management, medications, and healthy practices.
• Maintain accurate and timely clinical documentation in compliance with home health regulations.
• Identify changes in patient condition and report to supervising RN or physician promptly.
