ComForCare Home Health Care - Hilton Head is seeking a compassionate Home Health LPN to join our care team in Hilton Head, SC. You will deliver one-on-one in-home care, monitor patient health, and educate families on disease management and medications.

Qualifications

• Active LPN license in SC is required.

Responsibilities

• Monitor and document patient health status, vital signs, and progress toward care goals.
• Communicate effectively with patients, families, and the interdisciplinary care team.
• Educate patients and caregivers on disease management, medications, and healthy practices.
• Maintain accurate and timely clinical documentation in compliance with home health regulations.
• Identify changes in patient condition and report to supervising RN or physician promptly.

• Ability to work independently and manage time effectively in a field-based role
