Job summary

The Fortune Society, Inc. seeks a Dual Clinic Social Worker to join the Behavioral Health Services Unit at the Bronx Satellite.

You will provide therapy to clients with criminal justice histories and mixed caseloads of substance use and mental health diagnoses on-site, with occasional remote work and possible work at Long Island City. You will assess, plan and deliver individual and group counseling, maintain timely documentation, and collaborate with a multidisciplinary team to support reentry

Qualifications

LMSW, LCSW, MHC-LP, or LMHC license required.
CASAC credential a plus.
Minimum of one year clinical experience.
Bilingual in Spanish a plus.
SIFI certification a plus.
Experience with psychiatric issues, trauma or incarceration desired.

Responsibilities

Conduct initial client assessment and psychosocial interviews.
Develop a therapeutic alliance with clients.
Provide ongoing individual counseling and group counseling sessions when needed.
Write and update individual, client-based, goal-oriented treatment plans.
Maintain case records with notes within 48 hours of interaction.
Coordinate with supervisor and Fortune staff across programs/locations.
Link clients to outside services and participate in case conferences.
