Job summary

Aledade, Inc. is seeking a Director of Community Health Center Performance to lead data systems for CHC partner contract performance, driving high-quality, equitable care across Aledade’s CHC network.

The role focuses on performance management, identifying opportunities for improvement, and surfacing insights from data Analytics supports CHC performance improvements. The director will work remotely within the US with periodic travel, collaborating with CHCs, internal teams, and external partners

Qualifications

• Bachelor’s degree and/or 10+ years of healthcare work experience.
• 2+ years experience working with Federally Qualified Health Centers or in an FQHC.
• 5+ years of progressive value-based care experience.
• Demonstrated ability with population health analytics, including data systems and data visualization.
• Able to thrive in a fast-paced, startup environment.
• Demonstrated experience with taking an idea from concept to execution.
• Proven ability to manage multiple programs, coordinating across projects and teams.
• Strong and clear thinker/writer.

Responsibilities

• Collaborate with stakeholders to track performance (affordability, risk, quality, pop health impact) for CHCs on ACO performance.
• Develop and manage CHC data dashboards with support from BI.
• Leverage data insights to develop strategies to ensure VBC success via the CHC Center of Excellence.
• Support payer strategies designed for CHCs based on data.
• Participate in Collaborative Learning Networks and develop standard curriculums.
• Represent Aledade to CHCs during CHC summits and events.
• Other duties to ensure coordination across Aledade teams.
