Job summary

Empath Health seeks a Dementia Services Care Navigator to assess and support people living with dementia and their caregivers as part of the NRDEP team. You will provide outreach, facilitate caregiver groups, and develop referrals with physicians and clinics, ensuring access to dementia resources.

The role emphasizes counseling, crisis support, and public speaking to educate families and professionals, with field-based outreach requiring reliable transportation and a passion for Full Life Care.

Qualifications

• Master’s Degree in Social Work with emphasis on counseling, crisis management and community referral.
• Experience with dementia, elder care, caregiver needs and challenging behaviors preferred.
• Ability to speak publicly and engage families, healthcare professionals and first responders.
• Familiarity with facilitating small group learning and support groups.
• Reliable transportation for field-based outreach and education visits.

Responsibilities

• Provide outreach to professionals, organizations, and community members about NRDEP and dementia services.
• Develop referrals with physicians, dementia clinics and other providers; support caregiver groups.
• Facilitate Virtual Dementia Tours trainings as needed.
• Maintain knowledge of Medicare/Medicaid qualifications for skilled nursing facilities.
