Job summary

Adobe Population Health (APH) seeks a remote Nurse Practitioner or PA or MD/DO to deliver complex care via telemedicine for adults in CA. You’ll join the Population Health team, providing longitudinal, patient-centered care with a focus on chronic conditions, preventive and palliative care, supported by a field-nursing and admin staff.

This full-time role requires at least 3 years of clinical experience, CA licensure, and comfort with telehealth, care coordination, and addressing social

Qualifications

• Minimum of 3 years of clinical experience with a focus on complex care or population health.
• Active licensure as NP/PA/MD in CA with board certification preferred.
• Experience with telemedicine platforms and digital health tools.

Responsibilities

• Care Coordination: develop and implement individualized care plans with patients and family.
• Behavioral and Social Support: address social determinants of health and connect to community resources.
• Data and Quality Improvement: track outcomes and participate in QI initiatives.
• Collaboration and Communication: work with physicians, nurses, social workers and case managers.
• On-Call Requirements: participate in on-call rotation outside standard hours.
