Job summary

The University of New Mexico is seeking a Community Health Worker to assess and address clients' social needs and connect them with resources addressing SDOH. The role involves coordination with medical and social services to provide care coordination, education, and resource navigation in clinical, community, and home settings.

Remote work is eligible for this position, with training and certification provided.

Qualifications

• High school diploma or GED; at least 1 year of experience directly related to the duties and responsibilities specified.
• Completed degree(s) from an accredited institution that are above the minimum education requirement may be substituted for experience on a year for year basis.

Responsibilities

• Assess and address clients' social needs by developing actionable plans to connect individuals with resources addressing SDOH.
• Coordinate with medical providers, primary care teams, social services, and community organizations to deliver short-term care coordination, educational support, and resource navigation.
• Provide peer support in areas such as mental health, substance use recovery, housing instability, food insecurity, and other social encounters.
