Job summary

The Department of Health and Mental Hygiene (NYC DOHMH) is seeking a dedicated Community Coordinator to support surveillance, investigations, and outreach in congregate residential settings. You will conduct site visits, provide infection prevention guidance, and coordinate testing as resources allow in NYC.

Responsibilities include interviewing patients and contacts, performing data reviews, and developing outreach materials to prevent disease spread.

Qualifications

• Experience with communicable disease investigation and response.
• Experience working with congregate settings and populations.
• Background in health education and outreach, including presentations.

Responsibilities

• Notify congregate settings of cases or outbreaks and provide infection prevention guidance.
• Conduct site visits to implement public health interventions and assess infection prevention.
• Build relationships with NYC Health Department and NYS agencies overseeing congregate settings.
• Coordinate respiratory virus testing at facilities when resources exist.
• Data cleaning and quality assurance for outbreak documentation as needed.
• Plan and deliver education/outreach materials on prevention of communicable diseases.
• Support other BCD units with investigation and outreach as needed.
• Conduct communicable disease investigations, interviews, and chart reviews.
• Triage and respond to telephone inquiries on infectious disease issues.
• Participate in Incident Command System for emergency response as needed.
