Job summary

Icon Health is seeking a board-certified physician to serve as a collaborating physician for our Advanced Practice Provider team. The role supports licensing, credentialing, and collaboration requirements under Tennessee law.

The physician must live and practice physically in the state of Tennessee and be available by telephone or other electronic means when APPs are providing services. The collaborating physician will work with Icon Health’s APPs under the Practice Agreement, ensuring

Qualifications

• Board-certified in sports medicine, PM&R, physiatry or similar.
• Active Tennessee license in good standing.

Responsibilities

• Serve as collaborating physician for Icon Health APPs practicing in Tennessee.
• Assist with licensing, credentialing, CAQH updates, payer enrollment, and related admin tasks.
• Be available by phone or electronic means when APPs provide medical services.
• Provide clinical guidance and consultation when APPs encounter patient care needs.
• Support appropriate referral or escalation when clinically indicated.
• Participate in APP competency and qualification evaluation processes.
• Ensure APPs have the qualifications, training, and experience for authorized services.
• Review and update the Practice Agreement regularly and sign required documents.
