Job summary

The Centralized Order Entry (COE) Pharmacist will monitor, evaluate and optimize drug therapy for patient care within HCA Healthcare health system. You will enter orders, screen for drug interactions and maintain compliance with hospital policies and procedures.

Current California Pharmacist License is required; rotating days/evenings with weekends/holidays. This work-from-home opportunity emphasizes safe, evidence-based pharmaceutical care across facilities.

Qualifications

• Bachelor's degree from an accredited college of pharmacy is required.
• Pharm D is preferred.
• One year hospital experience is preferred.
• Meditech experience is preferred.
• License - State Board of Pharmacy Required.

Responsibilities

• Provide patient care activities to ensure safe and effective drug therapy.
• Enter orders accurately in the computer in a timely manner.
• Follow procedures for non-formulary/target/restricted/investigational drugs.
• Screen for drug interactions, allergies, order duplications and diagnoses.
• Address facility queues promptly.
• Investigate and report adverse drug events and medication incidents.
• Review and interpret physician orders using the patient profile.
• Monitor IV drug concentrations and rate.
• Document clinical interventions and follow-up as needed.
• Assess orders for age-specific appropriateness from neonatal to geriatric.
