Job summary

Equity Staffing Group is seeking Clinical Pharmacists to join a Clinical Engagement Services team supporting MTM, medication adherence, diabetes, polypharmacy, STARS adherence, and more. This is a remote opportunity, and Utah licensure is preferred.

Ideal candidates hold a PharmD with PBM/retail exposure, strong MTM expertise, and the ability to work independently in a virtual setting. Join a team focused on improving patient outcomes from any location within the U.S.

Qualifications

• Current, unrestricted Pharmacist license in your state of residence.
• Strong knowledge of MTM and disease-state management.
• 1+ year of remote/independent work experience.
• Strong communication, organization, and technology skills.
• PharmD, PBM/retail experience, and bilingual skills are a plus.

Responsibilities

• Conduct telephonic consultations and comprehensive medication reviews.
• Identify gaps in care, adherence concerns, drug interactions, and high-risk medications.
• Provide medication and disease-state education.
• Communicate clinical recommendations to patients and healthcare providers.
• Document interventions and recommendations accurately.

Preferred .... Looking for Pharmacists that are licensed in Utah

What you’ll do:

• Conduct telephonic consultations and comprehensive medication reviews
• Identify gaps in care, adherence concerns, drug interactions, and high-risk medications

• Communicate clinical recommendations to patients and healthcare providers
