Job summary

Arine is seeking a Clinical Pharmacist to deliver MTM and other clinical services via phone, helping optimize patient outcomes within Arine's platform. You will collaborate with Pharmacist Lead and Clinical Support to ensure quality care and documentation.

The role emphasizes patient-centered care, strong clinical judgment, and the ability to work in a fast-paced, metrics-driven environment. This remote-ready position includes on-site meetings and seasonal scheduling considerations.

Qualifications

• PharmD from an accredited college/university.
• Licensed Pharmacist in good standing (multi-state licenses a plus).
• Knowledge of CMS quality measures, Star Ratings, HEDIS, and Medicare Part C/D measures.
• Knowledge of chronic disease states across multiple therapeutic areas.
• Ability to review and assess prescriptions considering patient factors and drug interactions.
• Strong communication and motivational interviewing skills.

Responsibilities

• Engage patients by phone to deliver Arine's clinical services including MTM, CMRs, and TMRs.
• Review completed medication reviews and counseling by other users.
• Document patient data in Arine's software and prepare written/oral reports.
• Follow up with clinics, providers, pharmacies, and patients to resolve issues.
• Develop intervention strategies to optimize medication utilization and quality measures.
• Manage a consistent volume of patient interactions while maintaining productivity.
• Meet individual and team performance metrics.
• Adapt across clinical projects based on evolving needs.
• Identify opportunities to improve workflows and the platform via feedback and tickets.
• Provide daily sign-out and clear handoff communication.
