Job summary

Arine is seeking a licensed Clinical Pharmacist to join its Clinical Care team. This remote, patient-facing role leverages Arine's software to optimize medication therapy, conduct MTMs and reviews, and deliver core clinical services for health plans and patients.

Under Pharmacist Lead supervision, you will perform medication reviews, adherence counseling, collaborate with engineers to improve the platform, and guide the Clinical Support team.

Qualifications

• Doctor of Pharmacy (PharmD) from an accredited college/university.
• Licensed Pharmacist in good standing; multi-state licenses a plus.

Responsibilities

• Engage patients over the phone to deliver Arine's clinical services including MTM, CMRs, and TMRs.
• Review completed medication reviews and counseling by other Virtual Pharmacist users.
• Evaluate, document patient data in Arine's software; prepare written and oral reports on patient care and team progress.
• Follow up with clinics, providers, pharmacies, and patients to resolve issues and transitions of care.
• Develop intervention strategies to optimize medication utilization and meet quality measures.
• Manage a high volume of patient interactions while maintaining productivity and service quality.
• Meet individual and team performance metrics related to outreach, documentation, and outcomes.
• Collaborate across clinical projects and submit tickets to improve workflows and platform.
