Job summary

Medix, on behalf of a client, is seeking a Clinical Pharmacist to join a Clinical Care team in the San Francisco area. You will leverage cutting-edge software to optimize patient outcomes and provide expert medication consultations.

You will engage with patients and providers, coordinate care across clinics and pharmacies, and help improve software platforms while staying current on pharmacology developments.

Qualifications

• PharmD from an accredited college or university.
• Current, unrestricted Pharmacist license in Utah.
• Experience with ambulatory/outpatient chronic disease management preferred.
• Strong attention to documentation accuracy and reliability.
• Comfortable working independently in a fast-paced environment.
• Technical proficiency with cloud-based clinical platforms.

Responsibilities

• Engage patients over the phone to deliver MTM services, counseling, and education.
• Review and document patient data within proprietary software.
• Coordinate care with clinics, providers, pharmacies and patients.
• Develop clinical intervention strategies to optimize medication use.
• Manage patient, provider, and pharmacy interactions with high productivity and quality.
• Meet individual and team performance metrics.
• Adapt and rotate across clinical projects as needed.
• Identify opportunities to improve workflows and software platforms.
• Provide daily sign-out and clear handoff communication for continuity of care.
• Coach and support Clinical Care team members.
