Job summary

Saint Alphonsus Health System is seeking a Clinical Documentation Specialist to improve physician documentation quality and accuracy. The role involves working remotely with HIM, physicians, and nursing staff to ensure precise coding and documentation for severity of illness and risk of mortality.

Requirements include RN license (BSN preferred) and 3–5 years hospital clinical experience with strong medical/surgical background.

Qualifications

• RN license required; BSN preferred.
• 3 to 5 years hospital clinical experience; critical care or medical surgical background preferred.
• Ability to pass written clinical competency exam.

Responsibilities

• Review medical records to ensure accurate representation of illness severity and risk of mortality.
• Interact with physicians, HIM professionals, and nursing staff.
• Educate medical, nursing, and ancillary staff on documentation improvement.
• Audit for accuracy and follow up on cases.
• Develop plans for physician education on documentation improvement.
• Collaborate with coding staff to ensure discharge diagnoses reflect patient status.
