Job summary

Gateway Electronic Medical Management Systems, Inc. is seeking a Nurse Care Manager (RN/LPN) for a 100% remote role in Michigan. This Independent Contractor position involves telephonic patient encounters and coordinating chronic care plans for patients with multiple chronic conditions.

Two years of clinical experience and an active RN/LPN license in Michigan are required. The role offers flexible hours and potential to transition to full-time W2 employment with benefits.

Qualifications

• Graduate from an accredited School of Nursing with active RN/LPN/LVN license.
• Minimum of two years clinical experience in Med/Surg, Case Management, or home health care.
• Experience using Electronic Medical Records and Microsoft Office 365 tools.
• Excellent communication, time-management, and HIPAA compliance.
• Ability to work remotely with a stable internet connection.

Responsibilities

• Perform telephonic encounters with patients and develop detailed care plans within the electronic health record.
• Manage a patient panel with a focus on Chronic Care Management and preventive care.
• Possess independence and flexibility to set own hours as a 1099 contractor; potential transition to W2 employment.
