Job summary

PharmD Live is seeking a compassionate Licensed Practical Nurse (LPN) to remotely support patients in chronic care and population health programs. This role conducts telephonic and virtual encounters, delivers education on disease management, coordinates care with the multidisciplinary team to improve outcomes, and collaborates with pharmacists and providers to optimize patient engagement.

Ideal candidates hold an active New Jersey LPN license, with at least two years in ambulatory or telehealth

Qualifications

• Active New Jersey LPN license in good standing.
• Minimum two years of nursing experience in ambulatory/telehealth or chronic care management.
• Strong communication and documentation skills.
• Ability to escalate patient concerns per protocol.
• Proficiency with electronic health records and web-based clinical platforms.
• Ability to work independently with high productivity and quality.
• Dedicated HIPAA-compliant home workspace with reliable internet.

Responsibilities

• Engage patients via scheduled telephonic and virtual care encounters.
• Support CCM, TCM, and RPM programs.
• Educate patients on chronic disease management, medications, and wellness goals.
• Review patient-reported concerns and escalate as needed.
• Assist with referrals, follow-ups, and post-discharge outreach.
• Promote adherence to treatment plans and self-management strategies.
• Document accurately per CMS, payer, and organizational requirements.
• Collaborate with pharmacists and care team to improve outcomes.
