Job summary

Harris Computer is seeking a Remote Care Manager to handle a patient panel with a focus on timely patient encounters and quality care. The role requires a nursing background and strong documentation skills for HIPAA compliance.

Qualifications

• Must have nursing licensure (RN/LPN/LVN/BSN) and current state license with no disciplinary actions.
• At least two years of clinical experience in clinic, Med/Surg, Case Management, or home health care.
• Hands-on experience with Electronic Medical Records and Windows/Office 365 utilities.

Responsibilities

• Abide by the plan of care and orders from the practice.
• Provide prevention and intervention for multiple disease conditions through coaching.
• Develop positive patient interactions and improve revenue via billable Care Management episodes.
• Create detailed care plans for doctors and patients for prevention and intervention.
