Job summary

VytlOne, a specialty pharmacy company, seeks a Case Manager with retail pharmacy experience to support patient care and coordination from a hybrid/work-from-home setup. The role emphasizes patient filings, clinical assessments, and collaboration with pharmacists to ensure timely shipments.

Qualifications

• Education: High School diploma or equivalent and SBOP-registered/ certified tech as required.
• Experience: 1–2 years in a relevant role and any combination of necessary knowledge, skills, and abilities.
• Knowledge, skills, and abilities: proficient with MS Office/Outlook, data entry accuracy, strong organization and communication, ability to multitask and work under pressure.

Responsibilities

• Obtains copy of prescription drug card and files in electronic chart.
• Ensures accuracy of patient demographic information in pharmacy system.
• Sets up patient copay assistance and foundation support.
• Works orders daily to ensure efficient turnaround; informs patients/prescribers/sales as needed.
• Contacts patients to schedule initial shipments.
• Educates patients on device maintenance when directed by pharmacists.
• Completes Case Management Assessments for new referrals.
• Creates initial and ongoing clinical assessments to be reviewed by pharmacists.
• Creates delivery tickets and ensures correct shipment addresses.
• Obtains payment method from patients and records in CPR+ for future use.
• Marks deliveries as Ready to Fill with accurate dates.
• Sends status update documents to prescribers as needed.
• Answers incoming calls and manages queues.
• Manages SPRX Ques: Claims to Adjudicate, Benefits Verification, Patients to Contact.
