Qualifications

• Associate's Degree in Nursing is required.
• Active RN license is required.
• CCM certification or ability to obtain within 18 months.
• Preferred: BSN in Nursing.
• Experience in case management or utilization management in health care is a plus.

Responsibilities

• Act as liaison between patient, primary care physician, and other providers.
• Assess, plan, implement, coordinate, monitor and evaluate options for patient care.
• Document notes accurately and communicate effectively with patients and providers.
