Job summary

The position is remote eligible with preference for candidates in Iowa or South Dakota, requiring CCM certification within 24 months and strong MS Office skills for documentation and communication.

Qualifications

• Completion of an accredited nursing program.
• Active and unrestricted RN License in Iowa or South Dakota.
• CCM; must obtain within 24 months of hire.
• 4+ years of diverse clinical experience.
• Strong verbal communication skills; influences action.

Responsibilities

• Must be flexible to weekend/holiday hours on a rotational basis.
• Use critical thinking to recognize high-risk/complex conditions and anticipate needs.
• Identify barriers to care and design individualized care plans.
• Partner with members, families, providers and resources to coordinate care.
• Educate members on benefits and services and expected outcomes.
• Document care management activities accurately and timely per guidelines.
• Remain current with standards and guidelines pertinent to Care Management.
