Qualifications

• 5+ years nursing, occupational therapy, or social work experience.
• Bilingual English and French, both verbal and written.
• Direct care experience with strong clinical knowledge.
• Excellent written, verbal, and documentation skills.
• Strong organizational skills and ability to multitask.
• Proficiency with MS Office; EMR and SaaS familiarity is an asset.
• Multiple provincial licenses are an asset; good standing required.
• Flexibility for occasional evenings and varied shifts.

Responsibilities

• Serve as the initial clinical contact for members.
• Direct members to Teladoc services based on medical condition.
• Collect health histories and document relevant data.
• Provide ongoing clinical support and navigate the healthcare system.
• Collaborate with Case Coordinators and clinical team on progression.
• Use technology to ensure case coordination and data integrity.
• Review complex medical issues and ensure accuracy of summaries.
• Provide clinical expertise on diagnoses and treatment plans.
• Meet service standards for case progression and satisfaction.
• Maintain privacy compliance and liaise with providers as needed.
• Document casework clearly and timely for transparency.
