Job summary

Intermountain Health is seeking a qualified Patient Care Coordinator to provide operational support for high-risk, complex patients and to collaborate with care management teams, families, and providers. Responsibilities emphasize HIPAA compliance, documentation standards, and coordination with payers and community services while supporting system-wide improvements.

Qualifications

• Three years of experience in patient care, care management, transition/discharge planning, medical assistance, healthcare coaching, or patient care coordination.
• Excellent interpersonal and communication skills.
• Ability to adapt quickly as needs arise.
• Knowledge of available health resources.

Responsibilities

• Referral management: screen, monitor pending referrals and maintain workload data.
• Patient consultation: contact new patients by phone to introduce the program and schedule initial assessment.
• Transition management: ensure effective transitions of care between settings and contribute to process improvements.
• Documentation: complete timely, accurate medical-record documentation per guidelines and billing standards.
• Team collaboration: lead interdisciplinary care conferences and coordinate with care providers and payers.
