Job summary

myLaurel is hiring a Care Navigator, Level1 to be the critical first connection our patients have with the myLaurel care team. This fully remote role supports scheduling, education, and care journey navigation from home.

You will handle high-volume calls, coordinate in-home and virtual visits, educate patients on services, and maintain accurate documentation in EHRs while collaborating with clinical and operations teams.

Qualifications

• 2+ years in healthcare call center or patient-facing env.
• Proficient with EHR platforms (Epic/Athena/Cerner).
• HIPAA-compliant private workspace.
• Reliable high-speed internet (≥20 Mbps).
• High school diploma or GED.

Responsibilities

• Primary Point of Contact: Lead contact for patients and caregivers via high-volume inbound and outbound calls.
• Scheduling: Coordinate and schedule in-home and virtual care visits efficiently and accurately.
• Patient Education: Educate patients on myLaurel program services and set expectations for their care journey.
• Post-Visit Support: Manage post-visit needs, including equipment coordination, pharmacy requests, and clinical documentation.
• Systems Management: Maintain high-integrity documentation in our internal systems and EHR.
• Cross-Functional Partnership: Collaborate daily with clinical and operations teams to deliver a seamless, white-glove patient experience.
