Job summary

You will develop and coordinate personalized care plans, educate families on services, and collaborate with providers to ensure timely access to care. Requires a Bachelor’s degree and 2–4 years of related experience; nursing licensure preferred.

Remote within MI. Hours are Monday–Friday, 8am–4:30pm, focusing on advocacy, resource coordination, and outcome

Qualifications

• Bachelor’s degree and 2–4 years of related experience.
• Graduate from an Accredited School of Nursing if holding clinical licensure.
• Current state clinical license preferred.

Responsibilities

• Develops, assesses, and coordinates care management activities to provide quality, cost-effective healthcare outcomes.
• Educates members and families on services and benefit options available to improve access to care.
• Coordinates between member/family and care providers to ensure timely access to services.
