Responsibilities

• Evaluates needs, barriers to care, resources, and recommends the best care plan.
• Develops or contributes to personalized care/service plans with providers and resources.
• Provides psychosocial and resource support to members/caregivers and care managers.
• Coordinates between member/family and care team to ensure timely access to services.
• Monitors progress and collaborates with providers for plan revisions as needed.
• Collects and maintains member information to ensure regulatory compliance.
