Job summary

Upperline Health is seeking a Care Navigator to support a team of clinicians by delivering telephonic and back-office coordination for Upperline Plus patients. You will schedule visits, manage referrals, coordinate preventive care, and assist families with navigating the health system while maintaining compassionate, patient-centered service.

Ideal candidates have an associates degree or higher and healthcare experience, with bilingual English/Spanish preferred.

Qualifications

• Associates degree or higher required.
• BSW/CNA/LPN/MA highly required.

Responsibilities

• Conduct telephonic coordination of care for Upperline Plus patients and families.
• Coordinate scheduling of in-home, clinic, and telehealth visits.
• Act as liaison between patients, families and providers to ensure clear communication.
• Provide education and resources to support patient navigation of the healthcare system.
• Participate in weekly interdisciplinary team meetings.
• Support multiple chronic conditions coordination as needed.
