Cambia Health Solutions is seeking a Remote Care Management Nurse to support members from home across Oregon, Washington, Idaho or Utah. You will perform case management and care coordination using clinical judgment to promote quality and cost-effective outcomes.

Ideal candidates bring 3+ years of clinical care or case management experience, an active RN license, and eligibility for licensure in the four states. This role offers remote work options and a strong benefits package.

Qualifications

• Nursing or health/human services degree (Assoc or Bach minimum) or equivalent experience.
• Minimum 3 years of direct clinical care or case management experience.
• Active, unrestricted licensure in a U.S. state or territory; RN license required for medical care management.
• Eligible for licensure in Idaho, Oregon, Utah, and Washington.

Responsibilities

• Conducts case management activities, including assessment, planning, implementation, coordination, monitoring, and evaluation.
• Applies clinical expertise to ensure compliance with medical policy and guidelines.
• Collaborates with physicians and departments to resolve claims, quality of care issues, and necessary changes.
• Responds to inquiries professionally while protecting confidentiality.
• Provides documentation that aligns with performance standards and timelines.
• Coordinates resources and prioritizes assignments to meet goals and timelines.
• Monitors and evaluates the effectiveness of case management plans.
