Job summary

Cambia Health Solutions is seeking a Remote Care Management Nurse to support members from home in Oregon, Washington, Idaho or Utah. The role focuses on case management, disease management and care coordination with a patient-centered approach.

The position requires an RN license, a health/human services degree, and at least 3 years of direct clinical or case management experience. Strong communication, tech proficiency and collaboration with care teams are essential.

Qualifications

• RN license required for medical care management.
• Minimum 3 years of direct clinical care or experience in case management, utilization management, disease management, auditing or retrospective review.
• Nursing or health/human services degree (Associate's or Bachelor's minimum).

Responsibilities

• Conducts case management activities: assessment, planning, implementation, coordination, monitoring and evaluation.
• Applies clinical expertise to ensure compliance with medical policy and standards of care.
• Collaborates with physicians, internal/external customers and other departments to resolve issues and identify improvements.
• Provides accurate documentation and maintains confidentiality of sensitive information.
• Interprets policies and communicates complex topics effectively.
