Job summary

PacificSource is seeking a case management clinician to coordinate health plan benefits, assess patient needs, and facilitate services across the care continuum. Responsibilities include advocacy, data collection, care planning, and resource linkage within a HIPAA-compliant framework.

The role requires an active RN license (LPC/LMFT/LCSW options considered) with CCM certification within two years. Travel is minimal and benefits include comprehensive health plans and retirement options.

Qualifications

• Active, unrestricted RN license required.
• Licensure as LPC, LMFT or LCSW preferred/required.
• CCM certification required at time of hire or within two years.

Responsibilities

• Clinician Care Managers facilitate client wellness through advocacy, assessment, planning, communication, education, resource management and service facilitation.
• Collect and assess patient information to develop a comprehensive, individualized care management plan.
• Develop a care management plan that promotes appropriate utilization and cost-effective services.
• Link clients with providers and resources across the care continuum while ensuring safe, effective, patient-centered care.
• Maintain client privacy, confidentiality, health and safety following policies and regulations.
• Interact with PacificSource personnel to assure quality customer service.
• Assist employers and agents with healthcare resource questions.
• Practice and model effective written and verbal communication.
• Utilize lean methodologies for continuous improvement.
• Meet department and company performance and attendance expectations.
• Follow HIPAA privacy rules and confidentiality standards.
