Job summary

CVS Health Corporation is seeking a Care Management Associate for the OhioRISE program. This full-time telework position requires residency in Ohio and involves managing enrollment for child and family welfare populations, developing wellness plans, and coordinating services across care teams.

Responsibilities include triage, outreach, coordinating referrals, and maintaining detailed documentation in the electronic record.

Qualifications

• Must reside in Ohio.
• 2-4 years’ experience in healthcare field or working with foster, child and family welfare populations.
• Strong communication, telephonic and organization skills; able to manage multiple priorities with enthusiasm.
• Proficient with personal computers and MS Office Suite (Teams, Outlook, Word, Excel).
• Some in-state travel (5-10%) may be required; reliable transportation.
• Must pass CANS certification exam within the first 3 months of employment.
• Preferred: Motivational interviewing skills

Responsibilities

• Initial review and triage of members.
• Manage population health member enrollment for child and family welfare.
• Maintain a low tier member caseload.
• Develop wellness plans and coordinate community resources.
• Identify gaps in care and coordinate referrals to medical services.
• Document member calls in electronic records with detail for compliance.
• Support team members by researching information and solving problems.
