Job summary

LSMA Management, Inc. is seeking a Concurrent Care Coordinator in California to provide administrative and care coordination support for inpatient and post-acute management.

This non-clinical role liaises with hospitals, post-acute facilities, providers, and internal teams to ensure timely referrals and accurate documentation. The ideal candidate will have at least one year in healthcare environment and a HS diploma (associate degree preferred).

Qualifications

• High School diploma or equivalent required.
• Associate degree in healthcare administration or related field preferred.
• Medical Assistant Certificate encouraged.
• Experience with referrals, authorizations, or care coordination workflows.
• Experience with Medicare Advantage or Medi-Cal populations is a plus.

Responsibilities

• Support inpatient and post-acute care management workflows.
• Facilitate referrals and authorizations and track cases.
• Coordinate documentation and transitions of care.
• Act as liaison between hospitals, facilities, providers and internal teams.
• Ensure regulatory compliance and appropriate utilization of services.
