Job summary

Millennium Physician Group is seeking a Care Coordinator to support outpatient care management under the RN Care Manager. The role focuses on care coordination, transitions of care, chronic disease management, patient outreach, resource navigation, and documentation to improve outcomes and patient experience.

The position is primarily remote, with potential work from MPG offices or outpatient clinics. LPN or CMA/RMA certification and strong EHR skills are preferred for this role.

Qualifications

• Must have an active Florida LPN license or current CMA/RMA certification.
• Minimum 3 years of clinical healthcare experience in outpatient or care management preferred.
• Experience with electronic health records (EHR) required and strong documentation skills.

Responsibilities

• Assist RN Care Manager in implementing individualized care plans.
• Monitor follow-up appointments, referrals, and care plan goals.
• Coordinate services among PCPs, specialists, hospitals, and community organizations.
• Conduct telephonic outreach to patients and caregivers to address barriers to care.
• Support transitions of care and post-discharge follow-up activities.
• Educate patients on chronic disease management, medications, and preventive care.
