Qualifications

• Must have CMA/LPN/LVN credentials.

• Bilingual English and Korean proficiency required.

Responsibilities

• Manage a panel of patients (up to 225) via phone and electronic communication.
• Perform structured health assessments and follow patient care plans, recording interactions electronically.
• Create electronic alert escalations and notify physicians or nurses based on clinical rules.
