As a Care Advocate, you will work independently and remotely to assist patients with navigating their healthcare, while also fostering community connections. We offer competitive compensation and a variety of benefits including health coverage and a 401(k) plan.

Qualifications

• Experience in navigating high-risk chronic conditions or complex care management is a major plus.
• Strong computer skills with web and app navigation capabilities.
• Ability to communicate with various types of people and motivate them.

Responsibilities

• Execute person-centered care plans for patients with serious, high-risk illnesses.
• Support care coordination by managing provider availability and rescheduling.
• Conduct assessments to understand patients' social and cultural contexts.
• Assist patients in navigating the Pennsylvania healthcare landscape.
• Refer patients to supportive services and community-based resources.
• Maintain detailed logs of all interactions and ensure documentation meets CMS standards.
