Wider Circle is hiring a Care Advocate to support our care delivery program (WiderCare+), bridging clinical care and social needs for high-risk patients in New Jersey. You will work with clinicians and operations to promote health, build trust, and coordinate community resources.

The role requires 3+ years in community health, strong communication, and digital literacy. Remote team setup with occasional field visits and a competitive benefits package is offered.

Qualifications

• Excellent written and verbal communication skills and ability to motivate diverse people.
• Ability to navigate web-based and app-based systems with ease.
• Knowledge of New Jersey health landscape and social services.
• High school diploma or GED; valid driver's license and reliable transportation.

Responsibilities

• Execute PIN care plans for high-risk patients.
• Bridge care between patient and billing practitioner; coordinate provider availability.
• Assess social determinants of health and develop action plans.
• Navigate NJ healthcare landscape and coordinate transportation or telehealth access.
• Support CMS workflows and update patient charts with precision.
