Job summary

Aviary is seeking a Clinical Eligibility & Intake Nurse for remote, 1099 contracting work reviewing EMR records to determine eligibility for our virtual cardiac rehab program. You will also perform intake assessments by video when needed, guiding new patients through history, plan, and program benefits.

The role requires RN or LPN licensure, 3–5 years in cardiac care, and strong documentation skills. You will join a mission-driven team delivering care via telemedicine.

Qualifications

• Active RN or LPN license (required); multistate/compact license preferred
• 3-5 years of clinical experience in cardiac care, cardiac rehab, or chronic disease management
• 3 years of experience in Epic or another EMR, including chart review
• Working knowledge of cardiac conditions, procedures, and medications, including CHF
• Experience educating patients, older adults, in clinical or virtual care setting
• Fluent in English (written and verbal)
• Proficient with Zoom, Google Suite, and EMR systems

Responsibilities

• Clinical eligibility (majority of hours): Review patient charts in Epic and EMRs to determine eligibility using inclusion/exclusion list and decision tree
• Identify current cardiac rehab referral orders, or create orders when needed
• Communicate eligibility decisions to enrollment team with concise documentation
• Work eligibility queue within team turnaround targets to schedule patients promptly
• Complete eligibility rechecks and support special projects
• Escalate cases outside the decision tree to the Clinical Eligibility Lead
• Intake coverage (as needed): Conduct initial video assessments for patients enrolling in the virtual program
• Review medical history, prior procedures, and medications before/during visits
• Develop individualized treatment plans including nutrition, psychosocial, fitness, and co-morbidity assessments
• Educate patients on program benefits and risk factor modification
• Use goal setting and motivational interviewing to engage patients
• Document each visit accurately per clinical standards
