Job summary

Our client in the healthcare sector is hiring full-time Remote Care Navigators to support medically complex cardiac patients, primarily with CHF. This non-clinical role emphasizes telephonic outreach, care-plan support, CMS-compliant documentation, and cross-team coordination under RN supervision.

The position reduces avoidable hospitalizations and promotes patient self-management. Requires MA/CCM background, strong CMS knowledge, and US-based work eligibility for CST/PST overlap.

Qualifications

• Active MA certification or LVN/LPN license.
• Minimum 2 years of experience in care coordination, case management, or ambulatory care.
• Familiarity with CMS PCM, CCM, and/or TCM program requirements and documentation standards.
• Technologically proficient with care coordination software and/or EHRs.
• AI fluency and comfort using AI tools to improve efficiency.
• Based in the United States and authorized to work; time-zone alignment required.
• Excellent written and verbal English communication; strong phone presence.

Responsibilities

• Conduct structured telephonic outreach to CHF and complex cardiac patients.
• Maintain an assigned patient caseload using risk stratification to prioritize outreach.
• Complete initial assessments and follow-ups covering symptoms, medications, psychosocial status, and SDOH barriers.
• Support Transitional Care Management follow-up within 48 hours post-discharge with medication reconciliation and scheduling.
• Provide patient education on CHF self-management and evidence-based strategies.
• Monitor for signs of worsening conditions or care gaps and escalate to supervising RN.
• Review dashboards to address care gaps and coordinate with the care team.
• Document time, interventions, care plans, and patient goals per CMS standards.
• Maintain proactive communication with RNs, cardiologists, and PCP offices.
