Job summary

Seamless Assist is seeking a remote Care Navigator for Cardiac care management. This full-time, 40 hours/week role supports medically complex patients with cardiac conditions, primarily CHF, via telephonic outreach, care plan support, and CMS-compliant documentation.

Under RN supervision, you will coordinate across the care team, educate patients on self-management, and monitor for flags requiring escalation. US-based hours with CST/PST overlap required.

Qualifications

• Active MA certification or LVN/LPN license required.
• Minimum 2 years in care coordination or case management.
• Familiar with CMS PCM/CCM/TCM documentation standards.
• Proficient with care coordination software and EHRs.
• AI tools use to increase efficiency.
• Authorized to work in the United States; US time-zone overlap.
• Excellent written and verbal English communication.

Responsibilities

• Conduct telephonic outreach to CHF and complex cardiac patients.
• Maintain a caseload using risk stratification for outreach.
• Complete initial assessments and follow-ups on symptoms, meds, psychosocial status, and SDOH.
• Support TCM follow-up within 48 hours post-discharge.
• Provide CHF self-management education and evidence-based strategies.
• Monitor for worsening conditions and escalate to RN as needed.
• Review dashboards to address care gaps and document actions per CMS.
• Communicate with RNs, cardiologists, and PCPs to coordinate care.
