Job summary

Seamlessassist is hiring remote Care Navigators for cardiac care coordination. This full-time, non-clinical role focuses on telephonic outreach, care plan support, CMS-compliant documentation, and coordination with the care team to reduce hospitalizations.

Under RN Care Managers supervision, you will manage CHF patient outreach, document activities, and collaborate with cardiologists and PCP offices. Remote US-based work with flexible business hours and a $21–24/hour rate based on experience.

Qualifications

• Active MA certification or equivalent clinical credential.
• Minimum 2 years of care coordination or case management experience.
• Familiarity with CMS PCM/CCM/TCM program requirements and documentation standards.
• Proficient with care coordination software and/or EHRs.
• AI fluency and use of AI tools to work faster and more efficiently.
• Based in the United States and authorized to work (US CST/PST overlap).
• Excellent written and verbal English communication.

Responsibilities

• Conduct structured telephonic outreach to CHF and complex cardiac patients.
• Maintain an assigned patient caseload using risk stratification.
• Complete assessments and follow-ups on symptoms, medications, psychosocial status, and SDOH barriers.
• Support Transitional Care Management follow-up within 48 hours post-discharge.
• Provide patient education on CHF self-management and evidence-based strategies.
