Job summary

Seamless Assist is hiring a Remote Care Navigator to support cardiac patients in a non-clinical, telephonic role. This full-time, 40-hour/week position is remote across the United States, with CST/PST overlap required.

You will perform structured outreach, assist with care plans, and document per CMS standards while coordinating with RNs and clinicians. Requires MA certification or LVN/LPN license, 2+ years in care coordination, US work authorization, and English proficiency.

Qualifications

• Must have active MA certification or LVN/LPN license.
• Minimum 2 years in care coordination, case management, or ambulatory care.
• Familiar with CMS PCM/CCM/TCM program requirements and documentation standards.
• Proficient with care coordination software and EHR systems.
• AI fluency and ability to use AI tools to improve efficiency.
• Residency in the United States with US work authorization.

Responsibilities

• Conduct structured telephonic outreach to CHF and cardiac patients.
• Maintain assigned patient caseload using risk stratification.
• Complete initial assessments and follow-ups covering symptoms, medications, psychosocial status, and SDOH.
• Support CMS-compliant documentation and billing basics.
• Coordinate with RN Care Managers, cardiologists, and PCP offices.
• Document time, interventions, care plans, and patient goals per CMS standards.
• Escalate clinical concerns to supervising RN as needed.
