Job summary

Seamless Assist is hiring a Remote Care Navigator – Cardiac to support a medically complex cardiac patient population, mainly CHF, from a home-based role in the United States.

The role involves telephonic outreach, care plan support, CMS-compliant documentation, and coordination with RN Care Managers, cardiologists, and PCP offices. Must have MA certification and 2+ years in care coordination; AI fluency is a plus.

Qualifications

• Active MA certification or equivalent clinical credential required.
• Minimum 2 years in care coordination, case management, or ambulatory care.
• Must be based in the United States with CST/PST overlap in US business hours.

Responsibilities

• Conduct structured telephonic outreach to CHF and complex cardiac patients.
• Maintain an assigned patient caseload using risk stratification.
• Complete initial assessments and follow-ups on symptoms, medications, psychosocial status, and SDOH.
• Support CMS-based documentation and CMS billing practices.
• Coordinate with RN Care Managers, cardiologists, and PCP offices.
• Escalate clinical concerns to supervising RN as needed.
