Elevance Health is seeking a Therapy Services Specialist (Bilingual Spanish) to support home and community-based services virtually. The role engages in telephonic/virtual assessments, educating participants and coordinating with the MCO to tailor supportive services and equipment needs.

Candidates should have a college-level PT/OT program and a current license, with Spanish language proficiency. The position offers flexible virtual work with in-person training as needed, within a hybrid/virtual

Qualifications

• Requires graduate of a college-level program in physical therapy, occupational therapy, or an accredited two-year program for a Physical Therapist Assistant or Occupational Therapist Assistant.
• Current active, valid and unrestricted license or certification as a PTA or OTA in applicable state.
• Bilingual Spanish required.

Responsibilities

• Performs telephonic and/or virtual assessments to identify participants needs.
• Provides recommendations to MCO for type and hours of supportive services required.
• Conduct objective assessments for program participation to determine level of support and services.
• Obtain participant history to inform comprehensive assessment.
• Complete assessments annually or more frequently as needed per program requirements.
• Educate participants and MCO on options for home modifications, DME, assistive technology, or other adaptive equipment.
• Assist clinical team with recommendations for equipment and services as needed.
• Collaborate with support team to report observations and outcomes.
• Document all member encounters per documentation standards.
