What to Expect (Job Responsibilities):
- Conduct assessments and complete appropriate screening tools to identify behavioral health conditions early
- Provide accurate clinical assessments of mental and behavioral health conditions to inform treatment
- Collaborate with the care team regarding patient treatment goals and plans
- Manage psychosocial aspects of chronic and acute diseases, making referrals as necessary
- Assist patients in understanding their behaviors and developing healthier coping mechanisms
What is Required (Qualifications):
- Must have an active LCSW and be able to practice independently, with potential for multi-state licensure
- Excellent knowledge of behavioral medicine and evidence-based treatments for medical and mental health conditions
- In-depth knowledge of behavior therapy models and intervention techniques, including Motivational Interviewing and Cognitive Behavioral Therapy
- Strong problem-solving skills and effective communication abilities
- Compassionate and patient demeanor, with experience in telehealth and occasional in-home visits
How to Stand Out (Preferred Qualifications):
- Experience working in a value-based care environment
- Familiarity with chronic kidney disease and end-stage renal disease treatment protocols
- Previous experience in multidisciplinary treatment team settings
- Knowledge of social determinants of health and their impact on patient care
