Job summary

Indiana Health Centers, Inc. is seeking a compassionate Licensed Behavioral Health Professional for a full-time, remote role. You will provide integrated behavioral health services within the Primary Care Behavioral Health model to underserved populations in Indiana.

Responsibilities include assessment, diagnosis, treatment planning, and collaboration with PCPs. A Master’s in social work and appropriate Indiana licensure are required, with a strong emphasis on bilingual Spanish communication and

Qualifications

• Must be bilingual in Spanish.
• Master's degree in social work from an accredited university/college.
• LCSW, LMFT, or LCMHC licensure in Indiana.
• BLS certification (AED included).
• Active and clean Indiana driver's license, including state-mandated auto insurance.
• At least 1 year of experience, 3 years or more preferred.

Responsibilities

• Provide comprehensive assessment and diagnosis of behavioral health patients.
• Assist providers in recognizing and treating mental, psychosocial, and substance abuse disorders.
• Provide effective treatment planning and assist patients in achieving goals.
• Evaluate crises and apply appropriate interventions and referrals.
• Provide assessment, consultation, and brief intervention for psychological/psychiatric disorders.
• Maintain an active presence and communicate with Primary Care Providers during clinic hours.
• Available for same-day and scheduled initial interventions with patients.
